Microsoft Forms
BYU–Hawaii has an enterprise license to Microsoft 360, which means all employees and students have an account and can use Microsoft Forms. It also integrates and works well with other Microsoft applications.
Note that though using Microsoft Forms can be done using a BYUH account, the collection, transfer, and storing of this information may not be secure for personally identifiable information (see Collecting Confidential Information
- Go to Microsoft Forms
- Sign in using your BYUH credentials (same as Canvas, PeopleSoft, Workday, etc.)
- To begin a new survey, click “New Form”
- Type the title of your survey in the “Untitled Form” box
- A description of your survey, its purposes, or a message for your participants can be filled in within the “Form Description” box below
- By clicking “Quick Start” you will be offered a range of question formats including multiple choice, open-ended response, rating, specific dates, ranking, Likert, and Net Promoter Score
- Questions can be organized into relevant sections, such as demographic, when “Sections” is selected under the “Quick Start”
- Each form of question and response format can be customized to allow multiple answers or to require a response
- Adjusting your question order or deleting a question is possible by simply clicking on the question you want to correct
- To add another question in your survey, click “Add New Question”
- If you wish to change the presentation of your survey, browse the options under “Style” or “Templates”
- To preview and test your survey from a participant’s perspective on both a phone and computer, select “Preview”
- When your survey is ready, click “Collect Responses” and determine who you like to access the survey and how (Link, QR Code, Invitation, Webpage). These elements, as well as date availabilities for survey responses and limiting responses, can be adjusted in “Settings”
- To track your survey responses and analyses, click “View Responses”. For further analysis, click “Open in Excel”
- To stop collecting responses, click “Settings” and deselect “Accept Responses”
